Any person who is engaged in a field or endeavor related to health physics or whose interest in the profession would make their membership in the chapter desirable may apply to the executive council for election to membership. If the applicant is not a current member of the National Health Physics Society, sponsorship by a member of the Florida Chapter of the Health Physics Society is required.

Plenary: A membership with all rights and privileges. Plenary Annual Dues: $10.00. Plenary Application
Affiliate: Membership provides one general affiliate membership in the FCHP and Plenary membership for a designated company representative. The company name and website will be listed in the Affiliate section of the FCHPS website and included in the FCHPS newsletters. Affiliate Annual Dues: (See Below). Affiliate Application
Emeritus: A membership with all rights and privileges. No dues required, certain restrictions apply. Must be approved by the Executive Council.

For your convenience, the completed application form can be faxed and payments can be made via PayPal (www.paypal.com) by logging on to your PayPal account and sending a payment to the Chapter email address listed below. If you wish to mail your application and pay by check, please make your check payable to FCHPS and send it to the mailing address listed on the bottom of the form.
Annual Dues Payment:You can pay your annual dues by logging on to your personal PayPal account (www.paypal.com) and sending a payment to the Chapter email address: email@example.com
